The Crawford family has two children instead of three
Verity and Jeremy have three children in the book, two of whom die before Lowen joins the household. The movie gives them two children, Harper and Crew, and makes them twins.
Harper’s drowning becomes the single child death. Showalter said the smaller family was already in the script when he joined and concentrated the mystery on one child.

Lowen keeps the letter the book has her destroy
Both versions introduce Verity’s letter after her death. It claims that the manuscript was a writing exercise, not a confession, leaving Lowen with contradictory accounts of the same events.
In the book, Lowen destroys the letter. In the movie, she keeps it. Showalter confirmed that it is still in her pocket when the story ends. He said preserving the explanation about the writing exercise mattered to him even when earlier screenplay drafts offered different revelations.
The movie then returns to a recorded interview with Verity, giving her the final words after Lowen’s closing thoughts.
How it was adapted
Verity narrates the manuscript Lowen is reading
The movie adapts the novel’s main story through its final letter. Chapters Lowen reads become scenes narrated by Verity, who sometimes addresses the reader directly.
Added recordings of Verity’s public interviews give Lowen another account to set beside the manuscript and the silent woman upstairs. Michael Showalter described reading the manuscript as an intimate relationship for Lowen, something he wanted the movie to make visible.
